No. Just create an account, go to the servers page, click claim server, and follow the steps there.

I was looking at the claim page for one of the servers and all of the options to change name or date or something like that we "blurred out".

How do you also copy+paste a document from Microsoft Word to here? I hae the pic from GT on Word, and I don't know how to bring it here.

When you want to claim a server to be your own, Gametracker will ask you to change your server's name to "GameTracker". This proves if you are the server owner. You can change the name via RM/SM or your server host control panel (if you rent it). GT will then scan your server; find the name has been changed, and give you the rights to the server.
